In order for us to provide the best care for you, it is very important that you bring the following items to your visit:
- Insurance information and card
- Photo ID
- Physician’s Referral Form, if applicable
- New Patients: please download and fill out the patient demographics and medical history forms
- A list of your current medications
- Medical records from prior visits that you think may be helpful
- Any pertinent x-rays
